Considering Full Arch Dental Implants? Here’s What You Need to Know from a Surgeon Who’s Been There Since the Beginning

If you’ve been researching full mouth dental implants online, you’ve probably come across a range of terms—All-On-4NeoarchProarch, and Immediate Teeth Replacement, to name a few. While the names differ, they all describe a similar concept: using 4 to 6 dental implants to support a fixed full arch of replacement teeth.

What really separates one from the other is often the brand of dental implant being used. Some clinics use Nobel Biocare (the original developers of the All-On-4® concept), while others may use different systems.

So, what does this treatment involve?

In most cases, once we place the implants, we can attach a fixed set of teeth immediately or within a few days. This immediate transformation is life-changing for many of my patients. The surgery is typically “graftless”, meaning we often don’t need to add or build up bone, which can otherwise delay treatment.

Even for patients with significant bone loss, we have options like zygomatic and pterygoid implants that anchor into more distant, stable areas of bone. These techniques have allowed many people—previously told they weren’t suitable candidates—to finally receive fixed teeth.

Importantly, there’s no age limit for this treatment. I’ve treated patients well into their 80s who wanted to regain confidence and function. Medical conditions do need to be assessed, of course, but the vast majority of people are suitable candidates.

Patients consistently tell me how much more natural full arch implant bridges feel compared to traditional dentures. They don’t slip, don’t come out at night, and allow you to eat and speak more comfortably. The difference is not just physical—it’s emotional. I see people reclaim their smiles and, with them, a big part of their lives.

Do your research—it really matters

Here in Australia, we’re fortunate to have many highly skilled surgeons performing this type of treatment. That said, experience counts, especially with procedures as complex and transformative as this.

If you’re considering full arch implants, here are a few important questions to ask during your consultations:

  • How long has the clinic been doing this specific treatment?
  • Can they show you real patient results that have lasted 5–10 years?
  • What implant brand is being used, and is it readily available in Australia?
  • What are the long-term maintenance expectations and costs?

Maintenance is a big deal

I often meet patients seeking second opinions who were never properly informed about maintenance. This includes cleaning, follow-ups, and the importance of ongoing care. This is one of the main reasons I do not recommend dental tourism—even if the upfront cost seems appealing. Some overseas clinics use implants that are not supported here in Australia, which becomes a major issue if anything ever needs replacing or adjusting. And from an insurance and legal perspective, it’s risky territory.

Remember, full arch dental implants are a surgical treatment—not a cosmetic procedure like a hair transplant. You need regular follow-up during the healing phase, and ongoing care from someone who knows your case well.

My own journey with Full Arch Implant Surgery

I’ve been performing All-On-4® treatments since it was first introduced to Australia in 2004. I was fortunate to attend the very first training at the Brånemark Centre in Perth and have seen the treatment’s evolution over the past two decades.

Today, we’re spoiled for choice. There are excellent options in both implant systems and bridge materials, which allows us to tailor the treatment to fit different needs and budgets. While the “top shelf” options might not be affordable for everyone, we can often provide outstanding outcomes using alternative implants and materials. Every treatment is always guided by clinical assessment and individual goals.
